This three-page document details the approved Access Policy of the WA DNA Bank. It does not cover requests for access to the associated clinical and phenotypic data collections - such requests must be made directly to the data Custodians.
WA DNA Bank Access Application Form - External Researcher[0.6MB, PDF]
Researchers requesting access to WADB collections must complete a WADB Access Application Form in accordance with the guidelines listed in the above Access Policy. This one-page application may be filled in electronically, printed out and posted back to the WADB. Alternatively, you can save the completed form and email it to the WADB Manager.
WA DNA Bank Dispute Resolution Policy[0.1MB, PDF]
The scope of this Policy covers any dispute arising between a party who is using the facilities of the WADB, or participating in its activities, and the WADB.
